The Age-Friendly Health Care Package for State Multi-sector Plans for Aging is a grant-funded project to improve care for older adults. The project brings together three separate initiatives focused on hospital care: Geriatric Emergency Department Accreditation, Geriatric Surgery Verification, and Age-Friendly Health Systems (4Ms Framework). The goal is to support State Multisector Plans for Aging in achieving their healthcare objectives. The project is currently being implemented in Tennessee, where two hospital teams have been invited to participate in a Prototyping Learning Community to test the effectiveness of the Package concept.
The project aims to answer two main learning questions: Can hospitals successfully implement all three components of the Age-Friendly Health Care Package simultaneously, and should the implementation of the Package be pursued and promoted based on its value to hospitals, the state, and partners?
